You should print out and bring the new 6 paged I-693 form;

(Espanol) and the following to the office of the civil surgeon
(Traiga por favor los documentos siguientes con usted):
1. Your valid passport ( Su pasaporte válido ).
You must have some form of photo ID.
Usted debe tener cierta forma de identificación de la foto.
2. Your vaccinations ( childhood and adult immunizations)
Vacunas de niño o adulto que tenga.
Written proof is required. New requirements are effective
December 14, 2009. Bring a copy of your immunizations,
receive immunizations at our office or have additional blood
drawn for laboratory testing of immunity. Se requiere la
prueba escrita. Traiga una copia de sus vacunas, reciba las
immunizaciones en nuestra oficina, Tambien hacemos examen de
sangre para saber si uste es inmune.

3. If you are pregnant, we will need a letter from your OB
physician stating your due date.(Si usted es embarazado,
necesitaremos una letra de su médico de OB que indica su fecha debida.)

4. If you are breastfeeding, the CDC states that it is safe for
you to receive immunizations. You may prefer to discuss
this with your own physician prior to coming. Si usted
amamantando, la CDC indica que es seguro que usted reciba
inmunizaciones. Usted puede preferir discutir esto con su propio
médico antes de venir.
5. If you have had a previously positive tuberculosis skin
test or have been treated for tuberculosis, please inform
our staff and bring that documentation with you. You will
get a chest x-ray in our office. Si usted ha tenido una prueba

la tuberculosis, informe por favor a nuestro personal y traiga esa

T-SPOT blood test is the alternative to having the Tb skin
test. To make an informed decision, PLEASE READ the CDC
